Southwest Washington Association of Behavioral Health is an organization of behavioral health professionals committed to professional development and community education.
Through our website we provide a referral database to the community and other professionals. If you wish to be included on the website you must complete the Website release form and the Website information form.
When returning your application, please be sure to enclose the following:
- Completed Membership Application (download)
- Website directory release form.(download)
- Website information form. (download)
- A copy of your current license (Provider and Institutional Members only)
- A copy of your diploma (Provisional Members only)
- A signed statement that you are working toward licensure from your supervisor. (Provisional Members only)
- A copy of your professional liability insurance policy with the name of the carrier, insurance effective date, and expiration date.
- A check for annual membership dues for the 2009 calendar year should be made payable to SWABH in the amount of $60.00 for Provider, Institutional and Provisional Members and $15.00 for Student Members. For Emeritus Members, the fee is waived.

The online directory is updated two times a year in the months of November and May. To be included in the directory all membership materials must be submitted by October 1st for the November additions and April 1st for the May additions.
Please do not use SWABH directory for purposes other than SWABH events. To notify all members of something outside SWABH and for any other questions, please send your requests to swabh.board@gmail.com
